FBC Blood Test Explained: What It Measures and What Your Results Mean

The FBC, or Full Blood Count, is probably the most requested blood test in the world. If a doctor has ever taken your bloods, an FBC was almost certainly part of it. Yet most people are handed their results with a page of abbreviations, from Hb to MCV to neutrophils, and no real explanation of what any of it means. This guide fixes that: what an FBC measures, what each marker does, and how to make sense of high or low results.


What Is an FBC Blood Test?


A Full Blood Count measures the number, size and condition of the cells in your blood: red blood cells, white blood cells and platelets. Because these three cell types are involved in almost everything your body does, carrying oxygen, fighting infection and clotting blood, the FBC works as a broad first look at your general health.


It's used to help check for anaemia, signs of infection or inflammation, clotting problems and a range of conditions that affect blood cells. It's also a standard baseline test: many people have an FBC done yearly simply to confirm everything is where it should be. What Does an FBC Measure?


The 13 markers in an FBC fall into three groups. Here's what each one actually tells you, in plain language.


Red blood cell markers: oxygen delivery


Haemoglobin (Hb) is the protein inside red blood cells that carries oxygen around your body. It's the headline number of the whole test: low haemoglobin is the defining feature of anaemia, which commonly shows up in daily life as tiredness, breathlessness or pale skin.


Red Blood Cell Count (RBC) is the number of red cells in your blood. It usually moves in the same direction as haemoglobin.


Haematocrit is the percentage of your blood made up of red cells. Low values point toward anaemia; high values can reflect dehydration or the body making extra red cells.


MCV (Mean Cell Volume) measures the average size of your red cells, and it's one of the most useful clues on the report. Small red cells often point toward low iron, while unusually large ones can point toward low vitamin B12 or folate, among other causes.


MCH and MCHC measure how much haemoglobin each red cell carries and how concentrated it is. Together with MCV, they help narrow down the likely cause when haemoglobin is low.


White blood cell markers: your immune system


White Blood Cell Count (WBC) is the total count of your immune cells. A raised count often means your body is fighting something, most commonly an infection; a low count can follow viral illness or reflect other causes that deserve a closer look.


The FBC then breaks white cells into five types, called the differential. Neutrophils are the first responders, typically raised in bacterial infection. Lymphocytes lead the response to viruses. Monocytes are the clean-up crew, often raised during recovery and in longer-running inflammation. Eosinophils rise with allergies, asthma and parasitic infections. Basophils, the rarest type, play a role in allergic responses.


The pattern across these five is often more informative than the total count alone, which is why the differential is included as standard.


Platelets: clotting


Platelet Count measures the cells that plug damaged blood vessels and start clots. Too few can mean easy bruising or prolonged bleeding; too many can accompany inflammation, iron deficiency or, less commonly, conditions that need investigation.


What Do High or Low Results Mean?


Every FBC report shows your value for each marker alongside a healthy reference range, and the honest answer to 'what does my result mean?' is: it depends on the pattern, not any single number. A few principles will take you a long way.


First, one marker slightly outside range is common and frequently harmless. Hydration, a recent cold, hard training, your menstrual cycle and even the time of day can all nudge individual values. Lab reference ranges are built so that a small percentage of perfectly healthy people fall just outside them.


Second, patterns matter more than single values. Low haemoglobin with a low MCV suggests iron deficiency, which a Ferritin Blood Test can confirm. Low haemoglobin with a high MCV points instead toward B12 or folate. Raised neutrophils suggest bacterial infection; raised lymphocytes suggest viral. This is the kind of reading a clinician does with your report.


Third, significantly abnormal results, or mildly abnormal ones that persist across repeat tests, always deserve a conversation with your GP. An FBC is a screening and monitoring tool, not a diagnosis: it tells you and your doctor where to look next, and it does that job extremely well.

Treat them as a guide only: exact ranges vary between labs, and by sex, age and other factors, which is why your own report always prints the range your result was measured against. A few terms you might see on your report, matched to these markers: anaemia means low haemoglobin, polycythaemia means raised red cells, leucocytosis and leukopenia mean raised and lowered white cells, and thrombocytopenia and thrombocytosis mean low and high platelets. They sound alarming, but each is a description of a number, not a diagnosis in itself: the cause behind it is what your GP works out.


One practical note: some medications and supplements can influence individual markers, so mention anything you take regularly when discussing results with your doctor.


Do You Need to Fast Before an FBC?


No. Fasting is not required for a Full Blood Count, so you can book any time of day and eat normally beforehand. Fasting only comes into play when an FBC is combined with markers like cholesterol or glucose in a larger panel, in which case the test page will say so clearly.
